Compensation:
$85,000 – $130,000 annually
plus
$6,000 in first-year bonuses

What You'll Do

• 
  Manage a home health caseload throughout
  Woodbury, Cottage Grove, Hastings, St. Paul Park, and Landfall.
• 
  Complete OASIS assessments and individualized plans of care.
• 
  Provide skilled nursing visits, assessments, and patient education.
• 
  Coordinate care with physicians and the interdisciplinary team.
• 
  Help patients remain safe and independent in their homes.
• 
  Maintain accurate and timely clinical documentation.

What We're Looking For

• 
  Active Minnesota
  Registered Nurse (RN)
  license.
• 
  3+ years of RN experience
  (home health experience preferred).
• 
  Strong clinical judgment and ability to work independently.
• 
  Excellent organization and communication skills.
• 
  Reliable transportation for travel throughout the territory.
