Job Description

Job Title: RN Case Manager | Home Health Float | Keep Patient Care Moving Company: Home Health Care, Inc. Location: Greater Twin Cities Metro Area | 21-County Service Area Schedule: Full-Time | Monday–Friday, 8:00 AM – 5:00 PM | On-Call Rotation Compensation: $95,000 – $130,000 annually plus incentive visit opportunities To Stand Out Email Elena at elena.ehrlich@hhcare.net with your resume and a brief overview of your RN experience, home health or OASIS background , and what interests you about serving as a Floating RN Case Manager . Patients Can't Wait for the Perfect Staffing Situation. A patient is discharged from the hospital and needs home health tomorrow. A territory experiences a sudden increase in census. A Case Manager takes a leave. A team needs additional clinical support. The patients still need care. That's where you come in. As a Floating RN Case Manager , you'll provide experienced clinical coverage throughout our service area, stepping into territories where additional Case Management support is needed most. Your role helps us make sure one simple thing remains true: When a patient needs a nurse, we have a nurse ready to care for them. You're the Nurse Who Keeps Care Moving This isn't a backup position. And it isn't about filling holes in a schedule one visit at a time. You'll step into a territory, take ownership of an assigned caseload, and help provide the consistency patients and clinical teams need. Assignments will typically last approximately three months , allowing you to establish yourself within an area rather than bouncing between territories every few days. You'll learn the patients. Understand their plans of care. Work alongside the local team. Coordinate with physicians. Follow their progress. And help make sure their care doesn't lose momentum. When the need shifts somewhere else, you'll be ready to do it again. Why This Position Matters Home health doesn't stop because census changes. Patients don't stop needing wound care. Medications don't stop requiring management. Physicians still need updates. Families still have questions. Patients still need assessments. And someone still needs to coordinate everything happening inside the home. Your flexibility allows our clinical teams to respond when needs change without sacrificing the quality or continuity of patient care. That's the value of a great Float Case Manager. How Floating Works Your assignments will rotate throughout our 21-county Greater Twin Cities service area based on patient census, staffing, and clinical needs. Rather than constantly changing locations, you'll generally support a territory for approximately three months at a time . During that assignment, you'll: Manage an assigned home health caseload Become familiar with the patients and caregivers in your territory Coordinate care throughout each patient's episode Work alongside the clinical team supporting that area Help absorb census growth or temporary coverage needs Provide consistency while the territory's longer-term needs are addressed Then you'll transition to the next area where your experience can make the greatest impact. You're not randomly floating. You're being strategically deployed. What You'll Do As an RN Case Manager, you'll be responsible for the complete clinical picture of your assigned patients. You'll: Manage an assigned home health patient caseload Complete OASIS Start of Care, Recertification, and Discharge assessments Perform comprehensive nursing assessments Develop and update individualized plans of care Provide skilled nursing visits Perform wound care, medication management, and other skilled nursing services Educate patients and caregivers on diagnoses, medications, and treatment plans Monitor patients for changes in condition Communicate clinical concerns directly with physicians Coordinate with PT, OT, SLP, social work, and other interdisciplinary clinicians Help prevent avoidable emergency department visits and rehospitalizations Maintain timely and accurate documentation using Homecare Homebase (HCHB) A Few Perks We're Especially Excited to Offer $95,000–$130,000 annual compensation Additional incentive visit opportunities Full-Time, primarily Monday–Friday schedule Structured territory assignments typically lasting approximately three months Meaningful 1:1 patient care Independence and autonomy in the field Mileage reimbursement between patient visits Cell phone reimbursement for eligible clinicians Free access to our MedBridge Education Hub Licensure reimbursement when it's time to renew Annual scrub reimbursement Comprehensive medical, dental, vision, PTO, paid holidays, and 401(k) Experienced clinical leadership and interdisciplinary support We're Looking for the Nurse Who Can Walk In and Take Ownership Floating requires something different. You need to be comfortable entering an established territory and quickly understanding what needs to happen. You need to be able to organize a caseload. Prioritize clinical concerns. Build trust with patients. Communicate with physicians. Collaborate with a new team. And continue moving patient care forward. We're looking for: Active Minnesota Registered Nurse (RN) license Strong clinical assessment and critical-thinking skills Home health and OASIS experience preferred Confidence independently managing a patient caseload Strong organization and time-management abilities Excellent patient, caregiver, physician, and team communication Ability to adapt as territory assignments change Reliable transportation Willingness to travel throughout our service area based on coverage needs Why Home Health Care, Inc.? For more than 30 years , Home Health Care, Inc. has helped Minnesota patients receive skilled healthcare where many of them want it most: At home. As a locally owned, Medicare-certified home health agency, we understand that delivering great care requires more than accepting a referral. It requires having experienced clinicians ready to care for that patient. That's why this position is so important. When needs change, you help us respond. When census grows, you help us keep up. When a territory needs support, you help keep patients covered. And through every rotation, the goal remains the same: Make sure great patient care never misses a beat. Be the Reason There's No Gap in Care. New territory. New caseload. New team. But patients who need the exact same thing: A great nurse who shows up, takes ownership, and keeps their care moving forward. If you're an experienced RN who takes pride in being dependable, adaptable, clinically strong, and ready when people need you , we'd like to meet you. $95,000–$130,000 annually. Incentive visit opportunities. Structured territory rotations across our 21-county service area. Email Elena at elena.ehrlich@hhcare.net with your resume and a brief overview of your RN experience. Where there's a need, you'll make an impact.
