Full-Time | Monday–Friday, 8:00 AM – 5:00 PM | On-Call Rotation


Compensation:

$95,000 – $130,000 annually

What You'll Do


  • Float between home health territories throughout the Twin Cities based on patient needs.

  • Complete skilled nursing visits, OASIS assessments, and individualized care plans.

  • Coordinate care with physicians and interdisciplinary team members.

  • Support local teams during census growth, vacations, and coverage needs.

  • Deliver compassionate, one-on-one care that helps patients remain safely at home.

  • Maintain timely and accurate clinical documentation.


What We're Looking For


  • Active Minnesota
    Registered Nurse (RN)

    license.

  • Strong clinical judgment and adaptability.

  • Comfortable working independently in a field-based environment.

  • Home health experience preferred, but not required.

  • Reliable transportation and willingness to travel throughout the metro.
