Schedule: 12:00 PM – 8:30 PM ET | 1 weekend per 4-week schedule
Pay: $31.94–$43.92/hr (DOE; midpoint ≈ $39.93 at ~10 years)
Shift Differentials: +$2.50/hr Evenings | +$2.00/hr Weekends ( stackable to +$4.50/hr)
Sign-On Bonus: $10,000 (paid in 5 installments over 2 years)

A 280+ bed acute care hospital is seeking an experienced RN Case Manager (Evening Shift) to support utilization review, discharge planning, and care coordination across diverse patient populations including ED, Med/Surg, Telemetry, Neuro, ICU, and PCU
In this role, you will partner with interdisciplinary teams to drive appropriate level-of-care decisions, reduce length of stay, prevent avoidable days, and support safe, timely transitions of care. Reports to the Director of Case Management
Complete comprehensive clinical and psychosocial assessments and establish criteria-driven care plans with clear discharge goals.
Lead interdisciplinary huddles to coordinate with physicians, RNs, therapy, pharmacy, and social work.
Conduct medical necessity reviews at admission and throughout stay; escalate level-of-care or status issues when appropriate.
Manage observation status and convert to inpatient when indicated.
Perform concurrent/utilization reviews and document communications with third-party payers.
Identify barriers to care and throughput; resolve obstacles to decrease LOS and prevent readmissions.
Coordinate post-acute services, placement, and community resource referrals.
Document case management interventions and track/trend variances to support performance improvement.
Ensure compliance with regulatory, ethical, and quality standards; assist with education and protocol rollouts.
Active Florida RN license or Compact RN (must obtain FL license within 90 days if Compact).
3 years recent acute care RN Case Management experience OR
3 years acute RN experience (Med/Surg, Tele, Neuro, ICU, PCU, or ED) with strong UR/throughput skills.
Associate Degree or Diploma in Nursing.

Home Health or Insurance Case Management experience.
APRN license (if current and compliant).
BSN
Case Management / UR certifications (CCM, ACM, RN-BC)
Base Pay: $31.94–$43.92/hr
Differentials:
+$2.50/hr Evenings
+$2.00/hr Weekends
Combined +4.50/hr when shifts overlap
Sign-On Bonus: $10,000 total
$2,000 on 1st & 2nd paycheck
$2,000 at 6, 12, 18, and 24 months
