Job Description

Location: Tallahassee, FL (Leon & Gadsden Counties)
Schedule: 12:00 PM – 8:30 PM | Evening Shift | 1 Weekend per 4-Week Schedule
Pay: $31.94 – $43.92/hr (DOE; midpoint ≈ $39.93 at ~10 yrs)
Shift Differentials: +$2.50/hr Evenings | +$2.00/hr Weekends
(Stackable → +$4.50/hr for evening/weekend combinations)
Sign-On Bonus: $10,000 (paid in installments over 2 years)
Relocation Assistance: Case-by-case

A 280+ bed acute care hospital is seeking an experienced RN Case Manager (Evenings) to support utilization review, care coordination, and safe, timely discharges across diverse patient populations—including ED, Med/Surg, Telemetry, Neuro, ICU, and PCU.

You will partner with the interdisciplinary team to ensure appropriate level of care, reduce barriers to throughput, and improve quality outcomes. This role reports directly to the Director of Case Management

Key Responsibilities

  • Conduct comprehensive clinical and psychosocial assessments to develop effective, criteria-driven care plans.

  • Lead interdisciplinary rounds/huddles; collaborate with physicians, nursing, therapy, pharmacy, and social work.

  • Perform medical necessity reviews at admission and throughout stay; escalate level-of-care concerns appropriately.

  • Manage observation vs. inpatient appropriateness and convert status when indicated.

  • Complete concurrent/utilization reviews; document and communicate findings to payers.

  • Identify and mitigate barriers to care, throughput, and discharge; reduce LOS and avoidable days.

  • Coordinate post-acute placements, services, and community resource linkages.

  • Document Case Management interventions; track trends and contribute to performance improvement initiatives.

  • Support compliance with regulatory, ethical, and accreditation standards; assist with staff education and protocol rollout.

Qualifications

Required

  • Active Florida RN license OR Compact RN license (must obtain FL license within 90 days of hire if Compact).

  • 3 years recent acute-care Case Management experience, OR

  • 3 years acute-care RN experience (Med/Surg, Tele, Neuro, ICU, PCU, or ED) with strong UR/throughput skills.

  • Associate Degree or Diploma in Nursing.

Also Considered

(If acute experience requirements are met)

  • Home Health or Insurance Case Management experience.

  • APRN license accepted if current and compliant.

Preferred

  • BSN

  • Case Management or UR certifications (CCM, ACM, RN-BC, etc.).

Compensation & Incentives

  • Base Rate: $31.94 – $43.92/hr (experience-based).

  • Differentials:

    • Evenings: +$2.50/hr

    • Weekends: +$2.00/hr

    • Can stack for a total of +$4.50/hr.

  • Sign-On Bonus:

    • Total $10,000 paid as:

      • $2,000 on first/second paycheck

      • $2,000 at 6, 12, 18, and 24 months

  • Relocation: Case-by-case.
