+ approximately 35 miles from your home for on-call and weekend coverage


Schedule:

Full-Time | Monday–Friday, 8:00 AM – 5:00 PM | 1 Weekend Per Month | 1 On-Call Shift Per Week


Compensation:

$85,000 – $130,000 annually

What You'll Do


  • Manage a home health caseload throughout your assigned territory.

  • Complete OASIS assessments and individualized care plans.

  • Deliver skilled nursing care and patient education.

  • Collaborate with physicians and interdisciplinary team members.

  • Help patients remain safe, healthy, and independent at home.

  • Maintain timely and accurate clinical documentation.


What We're Looking For


  • Active Minnesota
    Registered Nurse (RN)

    license.

  • 3+ years of RN experience

    (home health experience preferred).

  • Strong assessment, communication, and organizational skills.

  • Ability to work independently while managing a field-based schedule.

  • Reliable transportation.
