A leading home health organization in Salem, Oregon is seeking an RN Case Manager to join its growing clinical team! Pay: $52.00 - $62.00/hour RN Case Manager Opportunity: Full-time, permanent, direct-hire position Monday-Friday, 8:00 AM - 5:00 PM schedule Guaranteed weekly pay Mileage reimbursement provided Relocation assistance available Comprehensive benefits package including medical, dental, vision, retirement benefits, and paid time off RN Case Manager Responsibilities: Complete patient assessments and develop individualized plans of care Monitor patient progress and revise care plans as needed Administer medications and treatments according to physician orders Educate patients and families regarding health conditions and care plans Coordinate care with physicians and interdisciplinary healthcare teams RN Case Manager Qualifications: Current Oregon Registered Nurse (RN) license required Minimum of one year of recent acute care experience required Current BLS/CPR certification required Valid driver's license and auto insurance required Home health experience preferred The Surrounding Oregon Community: Serves patients throughout Salem and surrounding communities Located in Oregon's scenic Willamette Valley with easy access to Portland and the Oregon Coast Enjoy abundant outdoor recreation, wineries, parks, and river activities Family-friendly area offering excellent schools, dining, shopping, and community events
