What You'll Do:

  • Assess client needs and develop individualized care plans to support optimal health outcomes
  • Coordinate with physicians, therapists, and caregivers to implement and monitor care plans
  • Adjust care strategies as needed based on client progress and changing needs
  • Educate clients and families on disease management, medications, and care best practices
  • Maintain accurate records and ensure compliance with all medical standards and regulations

What You Will Need:

  • Active MN Registered Nurse License.
  • Experience in home health, case management, or clinical care (Preferred).
