RN CASE MANAGER
We are seeking a dedicated Registered Nurse Case Manager to join our healthcare team. The ideal candidate will be responsible for coordinating patient care, providing medical support, and ensuring optimal patient outcomes.
Responsibilities
Maintain medical documentation / charting on all assigned Service Members, to include tracking medical. status and documentation on Service Member contact (email, voice call, or personal interview) in eCase in accordance with (IAW) AR 40-66 and unit Standard Operating Procedures (SOP).
Provide reports and documentation related to Service Member medical readiness factors to DSS and unit command teams as requested and required.
Provide care coordination on all assigned Service Members and maintain automated referrals per SOP and submit case processing, medical documentation, and profile concerns to military/contracted provider.
Coordinate Service Member treatment plans with appropriate medical facilities to include requesting specific exams and evaluations as needed.
Attend PHA events as needed.
Ensure Service Members referred by a provider are followed by the case management team. Manage Service Members care from the identification of the condition to when the Service Member is either considered medically ready or administratively separated including:
Obtain release of information (ROI) from Service Member.
Track medical appointments by communication with unit to ensure Service Member attendance and submission of follow up documentation.
Review and track health assessments (PHA, SRP) lab results for accuracy and completeness.
Educate Service Members on resources available to improve medical and dental readiness, including TRICARE benefits and possible Veterans Affairs eligibility. Service Members not turning in medical documents or not seeking medical care for a reported issue due to lack of insurance is an unacceptable excuse and shall be reported as noncompliance.
Communicate with delegated government personnel to track compliance versus noncompliance.
Adhere to all laws, regulations and policies applicable to Individual Medical Readiness (IMR) and deployment to include maintaining Health Insurance Portability and Accountability Act of 1996 (HIPAA) standards and guidelines.
Coordinate with Medical Readiness Non-Commissioned Officers (MRNCOs), military medical providers or their alternate to provide information to Service Members leadership concerning medical updates and counseling. Specific details shall be documented in eCase for the MRNCO to review.
Inform military medical provider of the Service Members status and furnish them with the mandatory information/documentation as received and reviewed. Build initial shell of temporary profiles based off civilian provider documentation within the E-PROFILE database within 5 business days of receiving civilian documentation.
Review all documentation and compose a narrative summary (NARSUM) of the information NLT 10 days following profile determination. Write the NARSUM in memorandum format IAW AR 25-50; and write the same information using a medical note in the Service Members electronic chronologic progress note in MEDCHART eCase module.
Review clinical treatment plans and document treatment plans as given from a Service Members treating Provider. The Nurse Case Manager shall be responsible for transposing treatment plans from clinical documentation into ADOP packets, INCAP packets, Convalescent leave packets, and Medical Board packets where applicable. If a Service Members clinical documentation is not specific about the proposed treatment plan, the shall call the Service Members treating Providers Office and speak with the treating Provider and/or nursing staff in order to obtain an appropriate treatment plan.
Document all case management notes in MEDCHARTS eCase, which all cases shall be assigned in.
Scan and digitally update all medical documentation received by the Case Management team to MEDCHARTs Health Readiness Reporting (HRR) module IAW AR 40-66 and State Surgeons Office guidance on indexing to various digital folders/repositories in HRR. The Administrative Assistant will perform the majority of the case file indexing and upload.
Add all medical documentation to the Soldier Treatment Record (STR) in paper form IAW AR 40-66. Follow proper PII and HIPAA laws for medical organization and management. When a Service Member is discharged from the IAARNG, the Service Members digital medical record file shall be validated IAW regulatory guidance.
Organize medical documentation IAW regulatory guidance for disability evaluation system medical board packet specifics. Each medical board packet has a different requirement for medical document organization. Nurse Case Managers and Administrative Assistants shall comply with guidance and procedure for composition of the individual medical board packets.
