Job Title: RN Care Manager
Location: Geneva General Hospital, Geneva, NY/ Floating to Soldiers and Sailors Memorial Hospital, Penn Yan NY
Geneva General Hospital is seeking an experienced RN Care Manager to coordinate patient care, utilization review, and discharge planning. This role collaborates with physicians, nurses, patients, and families to ensure appropriate level of care, timely discharge, and access to post-hospital services.
Key Responsibilities
Assess patient needs and develop care and discharge plans
Coordinate services with community providers and post-acute agencies
Participate in interdisciplinary rounds and collaborate with the care team
Perform utilization review and monitor length of stay using InterQual criteria
Identify barriers to discharge and promote safe, timely transitions of care
Educate and advocate for patients and families throughout hospitalization
Qualifications
RN license required
Minimum 5 years of healthcare experience
Prior Care Management/Case Management experience in a hospital setting preferred
BSN preferred
PRI/Screen certification or ability to obtain within 6 months
Strong organizational, communication, and computer skills
Additional Information
Ability to travel between locations as needed
Must maintain patient confidentiality and work effectively in a team environment
Exposure to bloodborne pathogens possible
