The RN Care Coordinator - LTC is a licensed Registered Nurse who assists the RN Clinical Managers in managing the clinical and administrative functions of the nursing department. The RN Care Coordinator - LTC ensures high-quality, Resident-centered care, regulatory compliance, staff education, and professional growth within the facilities. This position manages Resident care at both Juliette Manor and Peabody Manor.
Job Responsibilities:
- Monitors and manages Resident care at both Juliette Manor and Peabody Manor confirming compliance with quality and safety standards
- Supports the RN Clinical Managers in planning, organizing, and directing nursing services. Coordinates schedules to align with Resident needs meeting nurse-to-Resident ratios while maintaining labor efficiency to budget
- Monitor resources of appropriate supply use to align with the departmental budget goals
- Participates in the interview process and onboarding of nurses. Acts a Preceptor to nurses ensuring all competencies are met
- Continually train, educate, and provide mentorship for nursing Team Members
- Regulatory survey readiness and participation in QAPI activities
- Provides consultation and maintains positive relationships with Providers and other interprofessional team members, collaborating to problem solve and improve Resident care
- Ensure timely documentation and communication with Residents, families and their health care team
- Assist with new Resident admission including order entry and care planning
- Assist the Minimum Data Set (MDS) coordinator with the MDS process including covering assessments as necessary
- Demonstrates clinical expertise in the provision of care in the clinical specialty assigned, and performs all functions of the professional clinical nurse (RN), which are age appropriate, developmentally sensitive, and culturally specific
Qualifications
- Current Wisconsin RN Licensure
- BSN Completion: Upon hire, any non-BSN nurse with less than one (1) year of licensed Registered Nurse experience will be required to obtain a Bachelor of Science in Nursing (BSN) within five (5) years of hire. This education requirement is grandfathered for employees hired prior to February 1, 2026, who may continue to meet the requirement with an Associate Degree in Nursing (ADN)
- American Heart Association Healthcare Provider Basic Life Support (BLS)
- Minimum of three (3) to five (5) years of long-term care experience
- Strong leadership, communication and organizational skills
- Knowledge of CMS and state regulations; EMR proficiency
Physical Demands
- Ability to move freely (standing, stooping, walking, bending, pushing and pulling) and lift up to a maximum of fifty (50) pounds without assistance
- Job classification is exposed to blood borne pathogens (blood or bodily fluids) while performing job duties
Work Environment
- Frequent exposure to sharp objects and instruments
- Occasional exposure to moving mechanical parts, fumes or airborne particles, toxic or caustic chemicals, and risk of electrical shock.
- Occasional high noise level in work environment.
- Possible exposure to communicable diseases, hazardous materials, and pharmacological agents.
- Occasional contact with aggressive and or combative patients.
