Under general supervision, the RN Hospital Care Coordinator is responsible for facilitating and coordinating the care delivered to an assigned group of patients through multidisciplinary and patient/family collaboration to ensure quality and cost-effective outcomes are delivered within appropriate care coordination parameters. Coordination involves assessment, planning, support, and evaluation of patient care and related outcomes. Activities to be performed are facilitation of appropriate status and level of care, screening for high risk and transition of care needs, interviews with patient/family, progression of care facilitation with the healthcare team.
The Care Coordinator is an active contributor in the development of systems (e.g., care maps, clinical paths) to improve the care of assigned patient populations.

Essential Functions

  • Implement evidence-based plan with patient in coordination with healthcare team. Develop, modify patient care and treatment plans as indicated by patient’s responses and conditions. Provide safe, appropriate nursing care and patient education in an ambulatory clinic setting. Safely administer medications and treatments as ordered, according to policy. (10%)
  • Nursing Assessment – Collects comprehensive data pertinent to the patient situation. Assess the needs of individuals, families and/or communities to identify potential health or safety problems. Communicates findings to patient and appropriate healthcare team members. Data maybe collected as the result of a face to face, video, or telephone encounter. (10%)
  • Partners with patient and inter-professional healthcare team to review assessment and plan measurable goals and outcomes and strategies for achievement. Consult and coordinate with health care team members to assess, plan, implement and evaluate patient care. Advocates for patients and family in accordance with ANA code ethics and professional practice standards. (10%)
  • Support interdisciplinary healthcare team through supervision, education, and training of other clinic team members including medical assistants, educators and care navigators. The ACS RN is key participant in the health care team providing problem solving, escalation of patient care issues, outreach to patients and families, and coordination of services and care activities within the clinic setting and the community (10%)
  • Maintain accurate, detailed reports and records. (10%)
  • Prepare patients for, and assist with examinations and treatments (25%)
  • Nursing Diagnosis analyze assessment data to determine nursing diagnosis. (10%)
  • Direct and supervise less skilled nursing/health care personnel (5%)
  • Promote positive patient experience (10%)


Education

  • Associate's degree completion of a nursing education program that satisfied the licensing requirements of the Colorado State Board of Nursing for Registered Nurses required


Work Experience

  • 1-3 years of clinical experience in a hospital, acute care, home health/hospice, direct care or case management required AND
  • 1-3 years of experience in care coordination or case management required


Licenses

  • RN-Registered Nurse - DORA - Department of Regulatory Agencies required


Knowledge, Skills and Abilities

  • Knowledge and understanding of case management/coordination of care principles, programs, and processes in either a hospital or outpatient healthcare environment required.
  • Effective problem-solving skills, including the ability to analyze complex situations, draw conclusions and implement actions appropriately and efficiently.
  • Evidence of leadership, creativity, integrity, and initiative required.
  • Ability to work effectively in a team environment.
  • Experience with windows-based computer programs and ability to use computer for data analysis and data display required; must be able to type 35 words/minute.
