Saint Luke's Hospital is looking to add a RN, Care Coordinator to the Care Connections team.

The ideal candidate will have previous care coordination or case management experience in an acute care setting.
BSN Degree required within 3 years of start of position.
Shift: Full time Days
Care Connections Nurse is part of a team designed to effectively assist patients in transitioning from hospital to community. Care Connections Nurse will work with the patient to focus on collaboration between hospital and community-based providers and arranging post discharge services. Care Connections Nurse functions as a member of a multidisciplinary team with a goal of improving the patient’s health status and reducing the need for hospital care. Duties may also include assistance with program development, evaluation, professional education, or supervision of students.
