RN BMH Stomal Therapy/WOCN Clinical Nurse Coordinator FT Days
Under general supervision and in accordance with the policies, procedures and guidelines established within the organization, unit and the Nurse Practice Act, the RN manages the care delivery for patients in the hospital throughout the length of stay to achieve quality outcomes and timely discharge. Employees providing direct patient care must demonstrate competencies specific to the population served.
Bachelor of Science in Nursing degree from a four-year college or university and 2 years clinical experience required.
Licensed Registered Nurse in good standing with the State of Michigan
BLS required within six months after hire
Must obtain nationally recognized certification in wound and ostomy management, within 2(strongly suggested) years of taking the position and then maintain certification. CWOCN and Continence certification preferred.
Complete continuing education programs relevant to areas of practice.
Work which produces high levels of mental/visual fatigue, e.g., interactive and repetitive or small detailed work requiring alertness and concentration for sustained periods of time, the operation of and full attention to a personal computer or CRT between 40 and 70 percent of the time. Involves regularly lifting of bulky or moderately heavy weights (i.e., up to 50 pounds), and occasionally assisting with heavier tasks or expending the equivalent effort in pushing, pulling, or otherwise handling material, equipment, and other objects.
General Summary
The WOCN (wound, ostomy, continence nurse) Clinical Nurse Coordinator coordinates and provides consultative and direct clinical care services for ostomies, fistulae, tubes, dermal wounds, and come continence care throughout inpatient and outpatient areas, in both adult and pediatric (BMH specific) populations. Provides education services for nursing, medical and allied health staff, and consumers in regards to areas of expertise. Works in close collaboration with providers to establish patient treatment goals and plan of care, as well as monitor outcomes to adjust plan of care. Develops education materials, policies, procedures, in-servicing for staff development, and recommendations for wound/ostomy equipment for the system included under the umbrella of the WOCN training and expertise. Participates with nursing quality and management to develop evidence based practice initiatives in regards to pressure ulcer prevention and management.
Patient Care and Consultation:
Serves as primary educator and resource for patient undergoing ostomy or continent diversion surgery, through the continuum of care.
Provides consultation to staff regarding management of patients with established ostomy or continent diversion.
Provides consultation and assistance to staff in management of patients with draining wounds and fistulas.
Serves as consultant in the management of patients with urinary and fecal incontinence.
Provides consultation and assistance to staff in the management of patients with potential or actual alteration in skin integrity.
Educational Consultant and Mentor:
Orients staff to the WOC nurse role and the consultative procedure. Provides formal in-services on a routine basis and as needed to ensure current evidence based knowledge and practice. Develops and revises policies, procedures, and guidelines related to wound, ostomy, and continence care according to Evidence-Based Practice. Provides educational programs or mentoring for healthcare providers including nurses, providers, and students.
Serves as coordinator of own practice and as leader for nursing staff:
Collaborates with financial officers to establish and maintain a current charging system for WOC services. Analyzes stock items and recommends appropriate additions and deletions to assure quality and cost-effectiveness of products used for wound, ostomy, and continence care. Conducts product evaluations when indicated and submits reports and recommendations based on results. Contributes to research studies related to wound, ostomy, and continence care when feasible. Serves as agency-wide committees and participates in agency-wide quality improvement projects. Attends professional offerings, to further enhance clinical skills and stay current on clinical standards related to wound, ostomy, continence and skin care. Participates and in research related to WOC nurse practice including data collection, audits and interpretation of data.
