Job Description

We are a Central Pennsylvania based Non-profit Hospice & Post-Acute Healthcare Organization specializing in home-centered care and we are seeking a talented PRN RN Admissions Nurse.

Orientation: 2 weeks full time, 8am - 5pm + additional orientation over the next 16 weeks that can more flexible.

The RN Admissions Nurse provides prompt and efficient access and admission of patients referred to Hospice of Central PA. Provides explanations about the Hospice benefit and organization services in a manner consistent with the patient and family level of understanding. In this role, it is necessary to communicate, collaborate and enhance relationships with referral sources, with the overall goal of increasing admissions to the organization.

Qualifications:

  • RN, with current PA license
  • Minimum of one year clinical nursing experience—hospice or home care experience helpful but not mandatory
  • Excellent communication skills [written, verbal, listening]
  • Ability to track and complete multiple tasks at one time
  • Ability to work independently; self-motivated
  • Ability to work as part of a team
  • Ability to work in a non-judgmental fashion with people of diverse backgrounds
  • Actively supports the organization's philosophy of care.
  • Proficient at basic computer/keyboard skills
  • Knowledge and fluency of the Medicare Hospice Benefit, conditions of participation, industry and accreditation standards.

Functions:

  • In response to referrals, meets with patients and families to introduce the concept of hospice, explain the hospice benefit and facilitate admissions to services.
  • Makes routine visits to facilities to assist in discharge planning, making symptom management recommendations and collaborate with facility staff for the coordination of the plan of care for the patient.
  • Manages all aspects of care for clients under a General Inpatient Benefit (GIP) while in contracted facilities.
  • Facilitates a smooth transition of patients from inpatient to hoe or to the Hospice Residence.
  • Meets with targeted facility professionals to acquaint them with the hospice care philosophy and services provided by HCP.
  • Proposes new initiative to meet the needs of referral sources and improve patient access to end of life and palliative care.
  • Documents patient needs, interventions, recommendations in compliance with organizational policy and federal and state requirements as well as industry standards.

About Hospice of Central PA

Hospice of Central PA (HCP) was founded in 1979. The organization’s mission is provide the highest quality of emotional, physical and spiritual support with compassion and respect. HCP envisions a community that embraces specialized care at the end of life with the same importance that is given at the beginning of life.

Started by a small group of individuals concerned about care for the dying, our agency was one of the pioneers of the hospice concept nationwide and the first hospice in Central Pennsylvania. In the mid 1990’s we responded to patients requiring more support then they could get a home, but who still wished to remain in a home-like setting. We opened Carolyn's House, HCP’s hospice residence in Harrisburg in 1996. This six bed residence, located on a serene, wooded setting, is the first and only stand-alone hospice residential facility in Central Pennsylvania.

Our caring and supportive staff and volunteers serve all of Cumberland, Dauphin, Perry and Schuylkill Counties. We also serve northern York County and areas of Adams and Lebanon Counties. We care for patients and families in the most rural townships, as well as in the cities and surrounding suburbs and boroughs.

Hospice of Central PA is an independent non-profit, community based organization. We work closely with hospitals and other health care facilities. Our agency is a United Way Program Partner with United Way of the Capital Region (www.uwcr.org) and United Way of Carlisle and Cumberland County (www.carlisleunitedway.org). We also receive funding from the Carlisle Area Health and Wellness Foundation (www.cahwf.org). In addition, we enjoy a broad range of support from individuals, corporations, churches, private foundations and other organizations.
