The RI Works Case Manager provides comprehensive case management and care coordination services to families participating in RI Works and Project Opportunity programs. This role focuses on helping participants achieve employment goals, increase self-sufficiency, and access critical community resources through personalized, goal-driven support and service planning

Enjoy the flexibility of a hybrid work schedule, with remote work opportunities on Mondays and Fridays and community-based engagement throughout the week.

Key Responsibilities

Client Assessment & Service Planning
  • Conduct comprehensive assessments to understand client strengths, challenges, employment readiness, and family needs.
  • Develop individualized service plans with measurable goals and clear pathways toward self-sufficiency.
  • Monitor participant progress and adjust plans as needed to support successful outcomes.
  • Help clients navigate barriers that may impact employment or program participation.
Case Management & Care Coordination
  • Coordinate services across internal programs and external community partners.
  • Connect clients with resources related to housing, transportation, healthcare, financial assistance, and supportive services.
  • Advocate for clients and help ensure access to programs that promote stability and growth.
  • Build strong, professional relationships that encourage client engagement and long-term success.
Client Engagement & Support
  • Maintain regular contact with clients through office visits, home visits, and community-based outreach.
  • Deliver coaching, encouragement, and skill-building services that enhance employment readiness and daily living skills.
  • Support participants in meeting program expectations and achieving personal and professional goals.
  • Foster accountability while maintaining a strengths-based, client-centered approach.
Documentation & Compliance
  • Maintain accurate, timely, and complete client records.
  • Ensure compliance with RI Works, Project Opportunity, state, and organizational requirements.
  • Monitor caseload performance and productivity benchmarks.
  • Complete reports, case notes, and documentation according to established standards.
Community Outreach & Team Collaboration
  • Participate in supervision, team meetings, and ongoing professional development.
  • Support community engagement efforts and program outreach initiatives.
  • Collaborate with colleagues and community partners to ensure coordinated service delivery.
  • Contribute to a positive team environment focused on achieving meaningful outcomes for clients.

Required Qualifications
  • Associate's Degree required.
  • Minimum of two (2) years of experience in case management, human services, workforce development, or a related field.
  • Experience supporting individuals or families experiencing economic hardship, housing instability, or complex social service challenges.
  • Strong knowledge of community resources, public benefits programs, and service coordination practices.
  • Valid driver's license and ability to travel regularly throughout the community.
Preferred Qualifications
  • Bachelor's Degree in Human Services, Social Work, Psychology, or a related field.
  • Bilingual Spanish proficiency.
  • Experience working within RI Works, workforce development programs, or public assistance programs.
  • Familiarity with electronic case management and data tracking systems.

About East Bay Community Action Program

East Bay Community Action Program (EBCAP) is a private, non-profit 501 (c) (3) corporation. The mission of EBCAP is to provide high-quality, comprehensive and accessible health and human services to assist people to achieve their fullest potential.

In 2016 EBCAP merged with East Bay Center (EBC), a state recognized Community Mental Health Center (CMHC) and Integrated Health Home. Given that the two were recognized at the local, state, and national levels as premier providers, the transformation and merger into a single organization greatly enhanced the level of expertise and expanded the breadth and depth of services that EBCAP could offer to the communities it serves.

With this merger, EBCAP received provisional approval by the state as an Accountable Care Entity, an in-house partnership of medical, dental, behavioral health, and social service providers working together to create a shared culture of collaboration and transformation of the health care system. The ACE will focus on supporting high quality care, enhancing health information technology, and experiencing shared savings through agreements with major Medicaid providers, such as Neighborhood Health Plan of RI.
