Job Description

SpringPath Mental Health Services, a part of the Sevita family, provides behavioral health services through Intensive Residential Treatment, Assertive Community Treatment, and Targeted Case Management programs. We embrace a person-centered approach that is trauma informed, culturally responsive, and recovery-oriented. Our dedicated team supports individuals with mental health, psychiatric, or other complex needs to overcome challenges, build skills, and live a full, meaningful life.

The Resource Navigator works as part of an intensive residential treatment team that provides care, treatment and rehabilitation for persons served who are experiencing significant mental health/psychiatric symptoms; who are demonstrating significant functional impairments; and who may have co-occurring disorders.

ESSENTIAL JOB FUNCTIONS
To perform this job successfully, an individual must be able to satisfactorily perform each essential function listed below.

• Crisis Assessment and medical/behavioral emergency.
• Assists persons served in obtaining all financial, medical and community benefits for which they are eligible.
• Maintain current directory of a variety of community resources.
• Assist in discharge planning and care coordination process of the person served.
• Assist Clinical Director in obtaining prior authorizations from insurance companies.
• Collect rent from persons served or rep payee at the beginning of each month.
• Manage client invoices for Room and Board.
• Maintain an updated waitlist of referrals to facility, take and return phone calls about facility’s admissions process in a timely manner.
• Writes Progress notes and summaries.
• Group and Individual Counseling services.
• Skills Teaching.
• Case Management.
• Household management.
• Implements the treatment/rehabilitative activities and strategies.
• Crisis Prevention.
• Medication Administration.
• Participates in weekly staff meetings.
• Reports significant / major incidents or accidents.
• Establishes high levels of mutual trust and support with others.
• Provides coaching/learning environment.
• Rewards good performance and promotes employee recognition.
• Provides and promotes consumer choice and facilitate independence.
• Able to perform duties independently and as a team.
• Promoting the operation in a positive manner to the community and outside agencies.
• Promotes/encourages person’s served community integration and choice making.
• Dependable, accepts responsibility for attendance and punctuality.
• Conduct self in professional and ethical manner.

Minimum Knowledge and Skills required for the Job
The requirements listed below are representative of the knowledge, skill, and/or abilities required to perform the job.

Education and Experience:
• At least a bachelor’s degree in one behavioral science from an accredited college or university.
• Minimum of 2000 hours (post BA) of verifiable work experience providing services and support to individuals with mental illness.
• Or Master’s degree in one of the behavioral sciences from an accredited college or university.

Certificates, Licenses, and Registrations:
• Valid driver’s license and car insurance required.

Other Skills and Abilities:
• Case Management experience preferred.
• Ability to communicate verbally and in writing.
• Experience in managing systems, processes, and people.
• Operate general office equipment.
• Good organizational skills and attention to detail.
• Capable of working responsibly with highly confidential information.
• Must be able to work independently as well as part of a team.

Other Requirements:
• Travel as needed.

Physical Requirements:
• Medium Work: Exerting up to 50 pounds of force occasionally, and/or up to 30 pounds of force frequently, and/or up to 10 pounds of force constantly to move objects.

Pay Range: $55,000-$57,000
