The Resource Coordinator provides trauma-informed, guest-driven case management services to individuals experiencing homelessness, with the goal of helping them secure placement at a Homeless Resource Center (HRC) quickly and efficiently. This role focuses on identifying and engaging the most vulnerable individuals, building rapport, and facilitating their transfer into the appropriate resource center. The Resource Coordinator ensures a warm handoff between HRC case management teams and new guests to support continuity of care. Additionally, this position collaborates with shelter teams to identify common barriers faced by individuals who are not currently connected to a Homeless Resource Center and helps develop strategies and procedures to address those needs.

Essential Duties and Responsibilities

  1. Engage with the team to identify individuals staying at each location and assess their needs and barriers to accessing services.
  2. Conduct outreach and build rapport with the most vulnerable individuals in the community.
  3. Help the most vulnerable individuals transition from their current location intoto Homeless Resource Centers.
  4. Assist guests in identifying barriers and connecting them with appropriate resources in the community.
  5. Assist guests and their belongings in moving to Homeless Resource Centers as needed.
  6. Transport clients to Homeless Resource Centers using public transportation, cab vouchers, the Advantage Services Shuttle, and agency vehicles as necessary.
  7. Maintain an ongoing caseload of clients interested in seeking resources, assisting them with establishing goals, and eliminating housing barriers.
  8. Engage with all guests at St. Vincent's to connect them with resource centers, resources, and housing options.
  9. Follow agency guidelines to create and maintain healthy boundaries with guests.
  10. Collaborate daily with all departments to promote a supportive and effective team environment.
  11. Seek out, engage, and network with community partners to identify housing options and additional resources.
  12. Attend weekly team meetings and actively contribute to case-conferencing conversations.
  13. Maintain accurate records, detailed case management notes, and required statistical data; participate in special projects as needed.
  14. Provide direct, trauma-informed case management services to guests, including crisis intervention, harm reduction, outreach, ongoing assessments, goal setting, and creative problem-solving using a network of community resources.
  15. Respond to high-stress situations effectively using de-escalation techniques and trauma-informed practices.
  16. Assist with de-escalation and crisis intervention for guests experiencing crisis.
  17. Collaborate with the crisis clinician and other team members to support individuals with mental and behavioral health barriers.
  18. Attend mandatory agency trainings, including Trauma-Informed Care, Housing Focused, De-escalation, Suicide Prevention, CPR, and more.
  19. Participate in emergency drills and environmental safety activities, as required.
  20. Follow all agency protocols for security, safety, and sanitation to maintain a clean and secure environment for guests and staff.
  21. Maintain regular and reliable attendance as an essential function of this position.

Education and Experience

  • Experience working with diverse and/or vulnerable populations is preferred.
  • Lived, work or volunteer experience preferred OR a degree in a related field.

  11. Use basic computer programs (email, spreadsheets, and more) and have basic computer skills.

Physical and Equipment Requirements

  • Ability to lift and move heavy items up to 50 pounds, including guests' belongings.
  • Ability to sit, stand, and move for at least an hour at a time or more.
  • Ability to bend, squat, kneel, twist, push, and pull to perform job functions.
  • Ability to use stairs or steps.
  • Ability to safely climb stairs, ladders, and step stools.
  • Must be at least 21 years old, have a valid, unexpired driver’s license, and the ability to drive a vehicle, as needed.

Originally established as the Traveler’s Aid Society in 1923, The Road Home changed its name in 2001 to better reflect our mission of helping people step out of homelessness & back into our community. The Road Home is a private, non-profit social service agency that assists individuals & families with children experiencing homelessness in Salt Lake County & along the Wasatch Front. The agency has been providing emergency shelter services since 1988 & housing services since 1995.

We currently offer emergency shelter services in Salt Lake County with no eligibility criteria for clients to access services including; nightly beds, showers, access to laundry facilities, clothing, blankets, diapers, personal toiletries, etc. Case managers can assist clients in connecting with public benefits, substance abuse treatment, mental health assessment & treatment, job training/development, childcare services, housing assistance & more.

Our Pamela Atkinson (Men's) Resource Center, Gail Miller (All Genders) Resource Center & Connie Crosby Family Resource Center can be accessed 24 hours/day, 365 days/year. Priority populations like families with children, Veterans & those experiencing chronic homelessness are assigned case managers to facilitate transitioning out of shelter & into housing as quickly as possible. Case managers work with clients to develop a basic needs assessment & a short-term case plan. Once in housing, case managers continue working with clients to help them build the skills necessary to maintain stable housing, increase income & ensure access to mainstream benefits. This includes conducting in-home visits to ensure participants are meeting the terms of their lease, providing referrals to community resources & helping to identify and eliminate barriers that have prevented households from finding or maintaining housing.
