Job Description

Job Location: CARC - Lake City, FL 32025SUMMARY
Provide adults with developmental disabilities support and services as directed.

ESSENTIAL DUTIES AND RESPONSIBILITIES
Nurture a congenial, homelike atmosphere for all residents in the home.
Know and enforce house rules.
Become familiar with all residents goals, personal data and medical history.
Complete daily tasks on Shift
Sheet.
Assist residents with Activity of Daily Living Skills to promote independence.
Assist residents with daily household chores.
Assist residents with feeding, transferring, bathing, showering, dressing, and toileting as needed.
Document as required on Daily Communication Sheet, Residential Habilitation Training Data Sheets, Medical Administration Reports, Incident Reports, and any other required method of documentation. This includes but is not limited to recording progress, changes, concerns, incidents, res-hab training, activities, visitors, medication administration, and safety concerns.
Communicate to oncoming staff activities of prior shift.
Reflect flexibility, patience, and a positive attitude.
Ensure proper storage, handling, and record keeping for medications.
Provide prompt and appropriate attention to residents'/participants' medical conditions that require monitoring.
Consistently utilize all personal protective equipment provided.
Recognize and report signs and symptoms of abuse and neglect.
Provide supervision and transportation for medical appointments, work, and other social activities as assigned.
Demonstrate understanding of participant rights including privacy, confidentiality, and dignity.
Participate in In-Service Training.
Work schedule as assigned by supervisor, or as needed. (This may include shift work, rotating shifts, weekend shifts, etc.)

Other duties as assigned.
Promote compliance with Association rules and regulations. Work toward a consistent team approach with Association staff.

PHYSICAL DEMANDS
Position involves heavy work including lifting/transferring adults. Bending, balancing, twisting at waist, and push/pulling over 50 pounds. The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Good risk management must be practiced. Employee must utilize assistive devices and exercise extreme caution. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

SUPERVISORY RESPONSIBILITIES
Residents receiving group home services, program participants receiving respite, nonresidential supports and services, and companion services.

QualificationsQUALIFICATIONS

CERTIFICATES, LICENSES, REGISTRATIONS
Current CPR certification; Must possess a valid State of FL driver's license and maintain a safe driving record while in our employ. Must be medication certified within 90 days of employment.
EDUCATION and/or EXPERIENCE
High School Diploma or equivalent, plus one year experience with Residential Habilitation acquisition program(s) or experience transferring, lifting, bathing, dressing, toileting, and training individuals with disabilities (may substitute behavior management training with persons with developmental disabilities).
