Position: Program Coordinator

Reports to:Services Director

Program/Division: Residential

Supervises: Program Managers

Pay Classification: Exempt (Salaried, not eligible for overtime)

Program Coordinatorsassistin overseeing the daily operations of the assigned program to ensure the quality of services provided to supported individuals. This includes, but is not limited to, managing the program’s schedule, payroll, and finances, as well as the confidential safekeeping and management of supported individuals’ personal information.

Minimum Qualifications:

Eligibility for Employment

  • Ability to passan initialcriminal history check from the Department of Human Services Criminal Records Unit and recheck every 2 years thereafter.
  • Be a citizen of the United States or legally able to work in the United States.
  • Must be at least eighteen years old and hold a current driver’slicense, insurance, anda good drivingrecord.
  • Ability to complete required training and annual training thereafter.
  • The ability tomaintainregular attendance and to perform tasks outlined in this description (with or withouta reasonableaccommodation) is an essential function of this position.

Knowledge, Education, and Experience

  • HS graduate or equivalent required.
  • Two or more years of experience providing direct support to adults with disabilities, including hands-onassistancewith Activities of Daily Living (ADLs) such as hygiene care, dressing, bathing, and continence care
  • At least one year of formal management or supervisory experience, including responsibilities such as coordinating program operations, managing program finances, ensuring policy compliance, and supervising employees (e.g., assigning tasks, providing feedback, overseeing performance, orparticipatingin corrective action).

Duties & Responsibilities:

Supervisory Responsibilities

  1. The Program Coordinator oversees the time, attendance, and performance of one or more Program Managers.
  1. Interview, hire, andterminatestaff as needed (in conjunction with the management team and HR).

General Responsibilities

Overall Expectations:

  1. Be responsible forthe direct oversight and overall operation of the program(s) and ensure that the quality of life is in keeping with Home Life (HL) philosophy and values, policies, and procedures.
  1. Maintain accessibility for staff to share concerns, problems, and grievances.
  1. Share on-call responsibilities with the management team and provide direct support to supported individuals as needed.
  1. Manage all community-related issues that arise in relation to supported individuals and staff (in conjunction with the management team).
  1. Ensure that the Services Director and County are kept informed of allmajor issuesthat require follow-up or direct attention.
  1. Ensure that all pertinent informationregardingsupported individuals or the program is communicated as needed, including supported individuals’ whereabouts, incidents that have occurred, and required follow-up.
  1. Ensure that programs overseen are within the standards set by applicable OARs.
  1. Maintaineffectivecommunication withtheServices Director, Program Managers, DSPs, family members, and outside agencies asrequired.
  1. Delegate responsibilities/work assignments to Program Managers and DSPs to ensure that all required tasks are completed.
  1. Establish andmaintaineffective communicationregardingthe residential program with parents/guardians and supported individuals. Communicate medical, behavioral, and personalstatuschanges or concerns to parents/guardians, asappropriate,in a timely mannerwhilemaintainingconfidentiality.
  1. Ensure thatsupported individualsare informed of their rights and responsibilities to themselves, others, and the home annually.
  1. Coordinate with other programs as needed to execute company events andresolve problems.

Scheduling:

  1. Oversee coordination of staff schedules, including the scheduling of substitutes and volunteers, to ensure adequate coverage of the program. Meet weekly with theProgramManager toplanscheduling.

Meetings:

  1. Attendweekly meetings with the Services Director and provide reportsonprogram status.
  1. Conduct weekly meetings with Program Managers and receive reportsonprogram status.
  1. Oversee regularly scheduled staff meetings, in conjunction with the PM,at least twice per month.Ensure agency agenda items and pertinent information are communicated to PMs and DSPs during staff meetings.
  1. Attend all applicable program/agency meetings (staff, management, etc.).
  1. Participate in the supported individual Intake/Exit process asneeded,including all paperwork as requested by the Services Director.

Documentation:

  1. Assist the ISP Team with information related to legal forms orissuesas necessary.
  1. Write and update Individual Support Plans (ISPs) for supported individuals as needed and attend all corresponding meetings.
  1. Conduct and completestaff performance evaluations. Involve the PM in the process. (Review with the SD prior to presentation).
  1. Oversee theevaluation and documentation of performance of DSPs routinely.Ensure there is follow-up training if a staff person is not performing their job according to acceptable criteria.Give timely positive feedback verbally or in writingifa DSP is meeting or exceeding criteria.Submit all written feedback to the SD.
  1. Ensure that all department record systems aremaintainedat levels that meet required standards and policies.
  1. Set up a notebook for eachnewsupported individual within one week of their formal admission to the residential program.Perform monthly quality assurance checks to ensure maintenance of notebooks andTherapinformation.
  1. Ensure that all supported individual notebooks andTherapIDFs are continuously organized, updated, and complete. Update supported individual medical, financial, personal inventory, personal data, and personal contacts yearly.
  1. Review all program GERs and discuss incidents with PMs.Report GERs to the SD weekly and as necessary.Reportincidents to the County DD Program.
  1. Read all T-log entries and addfollow-upto staff logs as needed.
  1. Complete supported individual Monthly Change Reports, as needed.

Health and Safety:

  1. Ensure that the supported individuals’health and safetyaremaintained.

Finances:

  1. Oversee management of all program accounts, including Petty Cash, Activity Funds, Program/Food Budgets, etc. Perform mid-month and end-of-month cross-counts to ensure accuracy.
  1. Complete any paperwork related to Social Security, Medicaid, or food stamps.Ensure that theappropriate documentationismaintainedto complete this paperwork.

Training:

  1. Review to ensure the accuracy of in-service records.
  1. Ensure proper training and/or delegation of training ofnew staff, interns, and volunteers.
  1. Ensure that staff are oriented to the program/agency.
  1. Attend andfacilitatein-service training activities as assigned by theSD.
  1. Ensure that all staff in the program receive adequate in-service training each year andmaintaintheir certifications in required training,e.g., CPR, FA, MART, BBP, Core Competencies, and OIS.
  1. Oversee the design and implementation of supported individual training programs withappropriate staff.

Misc. and Other Duties as Assigned:

  1. Identifyand communicate cleaning and maintenance needs toSD.
  1. Assumeadditionalresponsibilities as assigned by the Services Director or Administration.

Physical Demands/Working Conditions:

The physical effort typically required in this job includes,but is not limited to, lifting up to 110 lbs.,performing assisted transfers involving supported individuals up to 250 lbs., pulling, reaching, repositioning, carrying, pushing, keying/typing, applying physical holds (as per Oregon Intervention System OIS instruction),and other duties as needed with or without reasonable accommodation.
