The Resident Service Designee supports the Resident Service Director in the administration of the nursing services. In conjunction with the Resident Service Director, the Resident Service Designee directs, plans, and coordinates services activities of professional nursing and auxiliary nursing personnel in rendering resident care. Interprets policy and regulations to all nursing personnel and ensures compliance. Analyzes and evaluates nursing and related services rendered to ensure quality of resident care. The Resident Service Designee in concert with the Resident Service Director assumes responsibility for providing oversight of the resident health care service’s needs, including the overall medication delivery system and the coordination of in-service training for the Resident Care Services Department. The Resident Service Designee contributes directly and positively to the team approach to quality resident care by attending to and assisting with a variety of physical, emotional and social needs to help residents maintain their highest level of independence possible.

Key Responsibilities:

  • Ensures clinical services are delivered in accordance with state regulations, accepted clinical standards, agency policies, and community procedures.

  • Provides direct nursing care, including health assessments, resident teaching, wellness counseling, health promotion, and disease prevention.

  • Delivers care and services to residents with chronic and stable conditions, supporting them in activities of daily living such as bathing, dressing, eating, toileting, hygiene, and mobility.

  • Verifies medication regimens with physicians or healthcare practitioners for residents requiring supervision.

  • Maintains awareness of, and compliance with, federal and state regulations regarding assisted living/memory care and the state nursing commission’s standards.

  • Leads and manages the delivery of nursing and personal care services for residents, ensuring excellence in care and adherence to clinical standards.

  • Collects and documents subjective and objective data during pre-move-in assessments, contributing to initial service plan development.

  • Participates in the move-in process, ongoing assessments, and service plan reviews to ensure appropriate care coordination and resident satisfaction.

  • Coordinates residents' healthcare in collaboration with residents, families, and external providers, including referrals as needed.

  • Conducts regular service plan reviews with residents and families, involving appropriate staff when care needs change.

  • Supervises and supports Resident Care Services associates, ensuring quality care delivery and proper documentation.

  • Delegates tasks in accordance with state regulations and community policy.

  • Coordinates and delivers initial job-specific training, ongoing education, and compliance with training standards through audits and observations.

  • Assists with staffing schedules and provides after-hours, on-call support for resident care needs, including possible off-site assessments.

  • Participates in infection control practices, immunization compliance, and infectious disease management in line with community policies.

  • Assists in managing transitions of care, including discontinuation of services when residents no longer require them.

  • Completes routine quality assurance audits covering all aspects of Resident Care Services, including documentation, staff performance, infection control, and delegation practices.

  • Prepares weekly reports for the Executive Director on issues related to core services and care coordination, contributing to reports required by the governing authority.

  • Participates in community-wide initiatives and other duties as assigned by the Executive Director.

  • Follows all emergency protocols and safety procedures as required.

Education/Experience/Licensure/Certification:

  • Must be a Licensed Practical Nurse or Registered Nurse, licensed to practice in the state.
  • Experience in Assisted Living, Home Health, Skilled Nursing and/or retirement center.
  • Experience and passion in working with seniors.
  • Experience in supervisory role and able to lead.
  • Able to perform tasks which may be physically demanding such as pushing, bending and lifting up to 60lbs.
  • Ability to maintain positive working relationships with residents, their families, peers and other staff members.
  • In good physical and emotional health and free of communicable diseases.
