Resident Care Coordinator (LPN/RN) – Assisted Living & Memory Care

Spring Oak Tri Cities is seeking an experienced Licensed Practical Nurse (LPN) or Registered Nurse (RN) to serve as our Resident Care Coordinator (RCC).

Our Petersburg campus consists of two buildings—one assisted living and one memory care. We are looking for an experienced nurse who can provide strong, hands-on clinical leadership while building relationships with residents, families, and the care team.

The RCC oversees resident care, clinical documentation, medication management, care planning, regulatory compliance, and the day-to-day supervision of the resident care team.

Qualifications

• Current Virginia Licensed Practical Nurse (LPN) or Registered Nurse (RN) license required.
• Previous nursing leadership, supervisory, charge nurse, or senior-level nursing experience strongly preferred.
• Experience in assisted living, senior living, memory care, dementia care, long-term care, skilled nursing, or residential care strongly preferred.
• Strong knowledge of resident assessments, care planning, medication management, eMAR/EHR systems, clinical documentation, and regulatory compliance.
• Strong leadership, communication, organization, and relationship-building skills.
• Comfortable providing hands-on support and leadership to the resident care team.
