Job Description

The Opportunity

The Resident Care Coordinator (RCC) is responsible to the Executive Director or the Director of Care. They ensure and support the implementation and evaluation of the resident care services in accordance with applicable provincial standards, guidelines, and regulations to ensure that the highest standard of resident care is delivered and maintained at all times.

What You Will Do

  • Provides support to all Care staff to attain and maintain quality care within a person-centered model of care.

  • Role models AgeCare Vision, Mission and Values.

  • Reviews trends on all incident reports and medication incident reports and ensures prompt corrective action to prevent recurrence.

  • Ensures a high standard of performance and a strong resident service orientation by providing regular feedback and ensuring compliance with all policy, procedures, and standards.

  • Ensures the completion of probationary and annual performance appraisals for all care staff in collaborations with the Executive Director and or Director of Care

  • Works collaboratively with HR Partner and the Executive Director or Director of Care to ensure concerns regarding resident services are investigated according to AgeCare policy and procedures.

What You Bring

  • Minimum LPN Provincial Certification required.

  • Must be fluent in to perform work in English, both orally and in writing

  • At least 5 years’ experience in as an LPN (or other Nursing designation) in continuing care setting, demonstration of leadership abilities with strengths in supervision, leadership, judgment, and interpersonal skills.

  • Experience in caring for Residents with Dementia.
