What You’ll Do:
The Resident and Family Life Assistant will assist the Resident and Family Life Manager to provide direction and leadership in developing, implementing and monitoring program goals and objectives for geriatric care management needed for Southminster’s senior adult community.
Supervises and coordinates the daily activities of maintenance staff and assigns work based on departmental priorities, staffing levels, and service needs.
- Assess and monitor residents in Embrace Health experiencing changes to develop and individualize care plans.  Work with the interdisciplinary team in conjunction with the resident and family in both internal and external settings. (IL, Healthcare, and Hospital settings.)
- Participation with admission process to determine appropriateness of admission, level of care issues, equipment needs and per diem rates.  Psychosocial assessments as needed.
- Acts as liaison to post hospital providers and insurance case managers.
- Tracks, monitors, plans, coordinates, or schedules activities on a complex level to ensure deadlines or requirements are met.
- Communicates on a complex level with others (internally or externally) to provide, exchange, or verify information, answer inquiries, address issues, or resolve problems or complaints.
- Facilitates communication between residents, families and staff.
- Advocates on behalf of the resident to ensure that care is coordinated.
- Maintains contact with the resident and, as appropriate, his/her social support system to facilitate the members participation in community life.
- Assist with on campus support groups for residents and/or families.
- Assist with monthly Resident & Family Life meeting to engage other departments to discuss/monitor residents who may need additional resources.
- Maintains resident confidentiality.
- Ability to provide services that support the individual’s broad spectrum of needs.  Advocates for resident’s quality of life.
- Maintain accurate and timely documentation in compliance with State and Federal regulations.
- Work with Healthcare Social Worker on short stays and transitions.  Ability to serve in Healthcare Social Work role.  Understands electronic health records and care planning process.
- Coordinate between interdisciplinary team members.

- Other duties as needed and assigned by Resident & Family Life Manager.

QualificationsBasic Requirements:
- Currently enrolled in or recent graduate of a Bachelor’s or Master’s program in social work, gerontology, sociology, psychology, or related field.
- 1 year in long term care, or completion of social work internship within a long-term care facility.
- This person must have excellent communication skills, organizational skills, energetic and self-directed.
