COMPANY PROFILE:
This physician-owned private practice has been operating independently and serving the greater Greenville area since 1997. They currently boast a staff of four (4) physicians and three (3) nurse practitioners, operating out of three (3) conveniently located clinics. Their services include, but are not limited to, primary care, cardiology, women’s health, and much more.
The Remote Patient Monitoring (RPM) Coordinator is responsible for monitoring patient health data transmitted from connected medical devices, engaging patients to support chronic disease management, and coordinating care with providers and clinical staff.
This role serves as a critical link between patients and the healthcare team by identifying potential concerns, promoting patient adherence, and ensuring timely intervention when abnormal readings occur.
Specific duties of the role include, but are not limited to:
- monitor daily biometric readings (blood pressure, glucose, pulse-ox, heart rate, etc.)
- review patient alerts and identify abnormal trends requiring intervention
- contact patients regarding abnormal readings
- provide education on proper device usage and medication adherence
- coach patients on healthy lifestyle habits (nutrition, exercise, disease management, etc.)
- escalate concerning clinical findings to providers according to established protocols
- assist with patient enrollment into RPM programs
- coordinate delivery, set-up, and troubleshooting of monitoring devices
- monitor patient compliance and encourage regular transmission of health data
- collaborate with providers, care managers, and support staff in continuity of care
- participate in QI initiatives aimed at increasing engagement and reducing hospitalizations
- stay current on CMS guidelines for RPM and organizational workflows
The practice uses the eClinicalWorks EHR.
REQUIREMENTS:
- active South Carolina RN, LPN, or CNA license
- strong customer service and patient engagement skills
- strong technical abilities
- understanding of RPM practices
- previous RPM experience preferred
- experience with chronic care management, primary care, cardiology, etc. preferred
