Responsibilities
• Evaluate and develop individualized treatment plans for patients.
• Deliver high-quality therapy services in accordance with professional standards and physician orders.
• Collaborate with interdisciplinary team members to maximize patient outcomes.
• Accurately document evaluations, treatments, progress notes, and discharge summaries.
• Educate patients, families, and caregivers on treatment plans and goals.
• Maintain compliance with regulatory, ethical, and organizational requirements.

Qualifications
Physical Therapist (PT)
• Current state Physical Therapist license in good standing.
• Graduate of an accredited Physical Therapy program.
• CPR certification preferred.
Occupational Therapist (OT)
• Current state Occupational Therapist license in good standing.
• Graduate of an accredited Occupational Therapy program.
• CPR certification preferred.
