Fixed term part time position (until Feb 2027)

What the role involves

We are seeking a skilled and enthusiastic Registered Nurse to join our close-knit and dynamic team. In this key role, you will undertake clinical assessments and coordinate the transition of patients into rehabilitation programs.

As the primary clinical point of contact, you will complete comprehensive assessments, collaborate closely with multidisciplinary teams, and support patients and their families at the start of their rehabilitation journey. Your expertise will help ensure timely, safe, and effective transitions of care.

This is a fixed-term, part-time position, with a regular Friday shift from 9:30am to 5:30pm

Key Responsibilities

  • Conduct detailed admission assessments for patients entering rehabilitation

  • Collaborate with medical, nursing, and allied health teams to support seamless care transitions

  • Provide education to referrers, patients, and families on rehabilitation goals and processes

  • Liaise with external providers to ensure continuity of care

  • Work flexibly across additional days and shifts as required

What we're looking for:

  • Registered Nurse with current AHPRA registration

  • Minimum 5 years’ experience in rehabilitation, acute, or sub-acute settings

  • Strong assessment, clinical reasoning, and critical thinking skills

  • Excellent interpersonal and communication abilities

  • Sound understanding of discharge planning, goal setting, and the Functional Assessment Tool

  • Ability to work both independently and collaboratively within a team

  • Current driver’s licence

  • Commitment to patient-centred care and continuous professional development
