Responsibilities:
The Rehabilitation Director is a licensed therapist who oversees the rehabilitation team and ensures the team is fulfilling the duties and responsibilities required to successfully operate within their facility.
- Oversees rehabilitation staff and ensure productivity standards are upheld daily
- Assigns daily patient schedules to therapy team while making sure daily list is current with any 24-hour changes within the facility
- Ensures caseload numbers are maintained at expected Medicare Part A and Part B numbers
- Orients new team members to the facility and educates on the roles and responsibilities of the home
- Ensures residents on caseload are seen by their applicable discipline by following physician’s orders as set forth in plan of care by evaluating therapist
- Attends daily/weekly/monthly meetings to ensure proper collaboration and communication with IDT throughout the facility
- Closely monitors therapy outcome data to identify trends, opportunities for growth and improvement, and to justify the development of new services/programs
- Coordinates closely with MDS coordinators to ensure information within skilled plans of care reflects PDPM data
- Ensures clarification orders are up to date, Common Working Files are obtained, documentation due is completed, and Part B tenth visits are completed on a weekly basis
- Completes month-end closeout to ensure cases are accurate for billing purposes
QualificationsQualifications- Must uphold state licensure as an evaluating or assistant therapist
- Must have strong leadership skills, be able to communicate clearly and professionally, and be adaptable to change and unexpected events that may arise throughout the day
- Must be able to maintain caseload with expected productivity while also balancing managerial duties
- Valid driver’s license, and an automobile in good working condition with appropriate insurance coverage in accordance with state laws
- Respectful, open communication with other team members, clients, and families
- Experience in a SNF setting and/or with rehabilitation director duties is preferred
