Job Shift: M-F Day Shift, Chartiers Center is a private, non-profit corporation funded by state, county, 3rd party insurances and private funds. Services include intake, referral, outpatient therapy, psychiatric assessment, service coordination, substance abuse treatment, training and social rehabilitative services, crisis intervention, partial hospitalization, community outreach, day and recreational services for adults with intellectual disabilities, homeless outreach and housing program.

• FULL TIME MONDAY- FRIDAY Daylight developing relationships with Providers in the Community.
• NO weekends, Holidays, On-Call shifts

This Registered Wellness Nurse is responsible for managing a caseload of individuals who have complex health and wellness needs, including those experiencing acute health events, unstable or progressive chronic medical conditions, barriers to accessing appropriate physical health, behavioral health, or substance use treatment services, and individuals who would benefit from health coaching, care coordination, and enhanced support in connecting with healthcare providers and community resources.

Job Duties

• Build positive, supportive relationships with individuals and assist them in achieving their self-identified health and wellness goals.
• Utilize evidence-based wellness coaching practices to empower individuals to actively participate in managing their health and care.
• Deliver services in settings that best meet the individual's needs, including homes, community locations, healthcare facilities, and other provider sites.
• Coordinate and, when necessary, facilitate transportation to medical appointments using agency vehicles.
• Review and analyze behavioral health and physical health utilization data to identify needs and develop individualized wellness plans.
• Monitor progress and provide follow-up support to promote adherence to treatment recommendations and care plans.
• Develop and implement preventive wellness strategies focused on areas such as nutrition, weight management, exercise, health screenings, and tobacco cessation.
• Connect individuals with healthcare providers, community resources, and support services to improve overall health outcomes.
• Maintain accurate and timely documentation in accordance with agency, regulatory, and funding requirements.
• Provide consultation and support to colleagues regarding the care and coordination of services for individuals served.
• Participate in educational activities and group-based learning opportunities that promote healthy lifestyles, wellness, and disease prevention.
• Serve as a resource to individuals, families, and staff on topics related to health promotion, wellness, and care coordination.

Qualifications

• Registered Nurse (RN) Certification from an Accredited Institution with PA licensure with 3 years behavioral health experience
• Valid driver's license required
