Services include intake, referral, outpatient therapy, psychiatric assessment, service coordination, substance abuse treatment, training and social rehabilitative services, crisis intervention, partial hospitalization, community outreach, day and recreational services for adults with intellectual disabilities, homeless outreach and housing program.
- FULL TIME MONDAY- FRIDAY Daylight developing relationships with Providers in the Community.
- NO weekends, Holidays, On-Call shifts
As a Registered Wellness Nurse with Chartiers Center, you would be committed to providing compassionate integrated health support to people facing mental health challenges in a community-based integrated care. This position will maintain a caseload of individuals who experience:
- An acute incident of a health condition
- A chronic health condition that is unstable or progressive
- A lack appropriate physical/behavioral/substance health care
- Will benefit from health coaching and/or lack appropriate supports with and linkage to health care providers.
Job responsibilities include:
Developing engaging and positive relationships with individuals and supporting them in self-identified goals.
Utilizing evidence-based practices related to wellness coaching and engaging individuals in managing their own care toward desired goals.
Providing services and supports in the location that is consistent with the needs and situation.  May include travel to other health providers, the home/community of the individual served and may also include using agency vehicles to facilitate transportation to needed medical appointments.  
Analyze individual and aggregate BH and PH care utilization data from BH and PH Managed Care Organizations MCOs) on these individuals as a basis for developing wellness plans. 
Provide follow up consultation and monitoring to ensure compliance with PH providers’ recommendations.
develop preventative wellness plans for consumers to preclude the development of chronic health conditions- i.e., weight management, nutrition, health screenings, exercise, and smoking cessation.
You will also provide supportive consultation to colleagues regarding the care of individuals and facilitate learning and/or consultative experience with consumer groups for the purpose of education, promoting health lifestyles.
Qualifications:
Registered Nurse (RN) Certification from an Accredited Institution with PA licensure with 3 years behavioral health experience
Valid driver's license
