About the Role
Monitor the provision of Elder care by the nursing team on an assigned neighbourhood, acting as the team leader. Assess the Elder’s needs in accordance with the InterRAI tool and provide a plan of care and interventions based on an evaluation of outcomes.

Duties and Responsibilities
Administration of Medications and Treatments:

  • Administer and document medications and treatments for all Elders, and assess effectiveness
  • Report Elder response to medication and treatments to physician/nurse practitioner
  • Transcribe and process all orders received

Communication and Documentation:

  • Chart care provided, behaviours and incidents
  • Respond to Elder and family inquiries
  • Educate Elders, families, and staff
  • Complete InterRAI assessments, CAPs and care plans according to schedule
  • Act as a mentor for students during clinical placement
  • Provide guidance and direction to Personal Support Workers, and other staff working on the assigned unit

Elder Care:

  • Assist as necessary with Elder care and transferring
  • Ensure Elder and family are comfortable during palliative phase
  • Monitor levels of liquid oxygen in portable tanks and fill as necessary

General:

  • Incumbents must keep up-to-date on all relevant legislation, collective bargaining agreements, County Policies and Procedures, etc. ensuring that these are implemented as prescribed
  • Incumbents must possess the physical ability to meet the duties and responsibilities of the job description and/or the requirements identified within the position's Physical Demands Analysis

What We Are Looking For
Minimum Formal Education

  • Registered Practical Nurse Diploma
  • Registration in good standing with the College of Nurses of Ontario

Experience

  • None required

Credentials

  • A clear Police Vulnerable Sector Check within the last 6 months
