Registered Practical Nurse

One of a network of provincial community health centres, the NorWest Community Health Centres is a community based, non-profit organization funded by Ontario Health, emphasizing access to primary health care and prevention programs and services.
We are currently seeking 1 FTE Registered Practical Nurse – Community Wellness Hubs, who is integral member of NorWest Community Health Centres’ interdisciplinary team, providing clinical services to individuals who experience barriers to accessing healthcare. This includes people who are underhoused, living with mental health challenges or diagnoses, experiencing current or past substance use, managing complex health conditions, identifying as 2SLGBTQIA+, newcomers to Canada, and individuals who are uninsured or underinsured.
This position provides integrated, team-based primary care services to clients, within social housing settings, while also supporting service delivery at designated community outreach locations as needed. In collaboration with internal teams and external community partners, the RPN supports individuals through a strong focus on disease prevention, health promotion, health education, and client empowerment.
The position has a competitive salary, the Health Care of Ontario Pension Plan (HOOPP) and an excellent group health benefit package.
The salary range for this position is $53,111 to $63,986 annually. We do not use AI to screen resumes.
Eligible for the Community Commitment Program for Nurses (CCPN) that provides $25,000 in grant funding to eligible nurses in exchange for a 2 year commitment. Click here for eligibility criteria.
QUALIFICATIONS:
• Registered Nurse (RN) in good standing with the College of Nurses of Ontario (CNO), with a minimum of 3–5 years of nursing experience.
• Experience working within a community-based care model and applying evidence-based primary care practices, including community outreach, health promotion, harm reduction, and the delivery of client-centred services in diverse community settings, is highly desirable.
• Demonstrated awareness, understanding, and sensitivity to the health needs of diverse populations facing barriers to care, as well as the socio-economic, cultural, and systemic factors that influence health outcomes, well-being, and access to services.
• Willingness and ability to work outside standard business hours, including evenings and weekends, based on operational needs.
• Strong organizational, communication, and leadership skills.
• Demonstrated critical thinking and creative problem-solving abilities.
• Ability to work both independently and collaboratively within an interdisciplinary team environment.
• Ability to perform clinical activities in accordance with College of Nurses of Ontario standards and professional nursing practice guidelines.
• Strong assessment skills to identify and address clients’ physical, mental, and psychosocial needs.
• Proficiency in electronic medical records (EMR) and electronic charting systems.
• Valid Ontario driver’s license.
