
The Registered Nurse (RN) Care Manager coordinates team-based care to provide health services to individuals, through effective partnerships with patients, their caregivers/families, community resources, their physician and healthcare team members. The RN Care Manager role also includes proactive, patient and family centered planning for patients’ progress that optimizes quality of care, patient satisfaction, utilization of resources and reimbursement to meet organizational strategic objectives.
Education Required Bachelor’s degree in nursing preferred.
Related Years of Experience Required: Three years’ experience in caring for chronic disease patients. Previous Care Coordination, Case Management or Home Health experience preferred. Previous experience with mobilizing community resources, navigating patients through the healthcare continuum, and working with disparate populations preferred.
Shift: Working 40 hours a week
