The Transfer Center Coordinator is responsible for partnering with Trinity Health EMS dispatch to coordinate, prioritize, and triage patient transports throughout West Michigan.
Trinity Health Grand Rapids is seeking an experienced Registered Nurse to join our Transfer Center as a Transfer Center Coordinator. In this critical role, you will serve as the centralized point of contact for all acute EMS patient transfers into, and across, our organization – ensuring safe, timely, and seamless transitions of care for patients and providers across the region.
As a Transfer Center Coordinator, you play a vital role in patient access and hospital flow. EMS interfacility transport is a complex, high‑risk component of healthcare delivery that is critical for patient access to appropriate care. Effective transfers depend on accurate triage, proper transport modality selection, and pre‑established coordination between facilities. You will collaborate with hospitals, physician offices, long-term care facilities, emergency departments, and internal clinical teams to ensure patients are transferred at the right time, to the right place, and at the appropriate level of care.
What you work work:
Full-time
Three 12 - hour shifts per week
Night Shift: 7:00 p.m. - 7:30 a.m.
Every third weekend rotation
Coordinate and manage EMS interfacility transfers for inpatient and emergency spaces.
Collaborate with Trinity Health EMS dispatchers to prioritize and triage patient transfers.
Assist in prioritization of facility discharge transports, including discharges to skilled nursing facilities, assisted livings, and inpatient psychiatric care.
Collaborate with Clinical Resource Coordinators on discharge transport needs to streamline patient flow and bed management throughout the facility.
Collect, document, and communicate critical clinical and logistical patient information.
Support ED-to-ED transfers and continuity of care.
Identify barriers to patient flow and escalate concerns appropriately.
Participate in quality and process improvement initiatives (PDCA, LEAN).
Deliver outstanding customer service aligned with Excellence in Care Experience principles.
Bachelor’s degree in Nursing or related health field
Current Michigan Registered Nurse (RN) license
Active BLS certification
Minimum of 5 years of progressive acute care clinical experience
Strong communication, coordination, and problem-solving skills
Master’s degree in Nursing or related field
Professional nursing certification
Experience with utilization management or process improvement
Epic EMR and standard computer application experience
