$75,000 – $110,000 annualized (per-visit model based on volume)

What You’ll Do
• Provide
skilled nursing visits
in homes and community settings.
• Perform
assessments, wound care, and medication management
.
• Educate patients and families with clarity and compassion.
• Collaborate with RN Case Managers and the care team.
• Float to nearby territories to support patient needs.
• Maintain
accurate, timely documentation
.

What We’re Looking For
• Active Minnesota Registered Nurse (RN) license.
• Strong clinical judgment and independence.
• Comfortable working in a
field-based role
.
• Home health experience preferred, but not required.
• Reliable transportation.
