As a RN Navigator in our Neurology Clinic, you will play a vital role in delivering comprehensive, patient-centered care coordination when we open our doors in February 2027. In this position, you will work alongside a dedicated multidisciplinary team to guide patients and their families through every step of the neurological care journey, from initial screening and diagnosis through treatment and beyond. You will serve as a trusted advocate and resource, helping to identify and overcome barriers to quality medical and psychosocial care while ensuring that patients and families are informed, supported, and empowered throughout the process. Through collaboration with physicians, specialists, and community providers, you will coordinate timely referrals, consultations, and services that reflect a holistic approach to care, all while contributing to continuous process improvement and the highest standards of clinical excellence.
Schedule: Monday - Friday 8am-5pm
Banner University Medical Group is our nonprofit faculty practice plan associated with the University of Arizona Colleges of Medicine in Phoenix and Tucson. Our 1,100-plus clinicians provide primary and specialty care to patients at highly ranked Banner - University Medical Centers and dozens of clinics while providing mentorship to more than 1,200 residents and fellows. This position provides comprehensive care coordination, supporting a holistic and coordinated approach across the continuum of care. This position assists with informed decision-making; collaborating with a multi-disciplinary team to allow for timely screening, diagnosis, treatment and increased supportive care throughout the patient experience. This position provides individual assistance to patients, families and caregivers to help identify and overcome barriers which may hinder quality medical and psychosocial patient care.
CORE FUNCTIONS
Must possess knowledge as normally obtained through the completion of a Bachelor's degree in nursing or related field.
Requires a current RN license in the state of practice. BLS certification required.
Requires a proficiency level typically achieved with 5 years clinical experience. Requires excellent organizational skills and clinical knowledge regarding specialty care services, as well as care coordination of services, legal and financial aspects of diagnostic services and health services in specialty area. Requires effective communication and writing skills, good time management skills and knowledge of word processing and database software applications. Requires the ability to teach both clinical and non-clinical personnel regarding care and diagnostics services. Also requires a good understanding of process improvement.
Current certification in specialty area preferred. Prior Case Management experience preferred.
Additional related education and/or experience preferred.
