
As a Long-Term Care Registered Nurse, you will provide high quality and compassionate care to patients, residents and families in your unit while supporting continuity of care across all shifts.

RESPONSIBILITIES
Patient Care & Service. Promote and restore patients' health by completing the nursing process; collaborating with physicians and multidisciplinary team members; perform various treatment procedures; provide physical, educational and emotional support to patients, friends and families; supervise assigned team members
Planning & Communication. Develop and document individualized care plans customized for each patient’s unique needs, with support from the interdisciplinary health team as needed; maintain effective communication to convey patient health status, treatment plans and progress
Team Management. Assume responsibility and accountability on the unit and for the patient care team, work collaboratively with others
Compliance. Adhere to organizational policies, procedures and protocols, including policies governing safety, medication administration and documentation, complete required number of continuing education credits per year and mandatory in-services and health screens
Minimum Qualifications
• Diploma or Associate’s Degree in Nursing required; Bachelor’s Degree in Nursing preferred.
Required Licensure/Certification Skills
• Registered Nurse license in New York State.
• Basic Life Support (BLS) certification.

PHYSICAL REQUIREMENTS:
M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.

The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant.
