Registered Nurse (RN) Hospital Liaison, Manager

Schedule: Monday - Friday 8:00am - 4:30pm

HOW YOU'LL MAKE A DIFFERENCE:As a Post-Acute Care Coordinator, Manager the work you do every day makes a difference in the lives of our patients by providing patient healthcare coordination services. You will lead a team of Post-Acute Care Coordinators (PACC) that assist the patient in transition of care from one setting to the next within the health system.

HOW YOU'LL WORK:You’ll provide oversight for your team and ensure eligibility for potential home care patients through knowledge of insurance, Medicare guidelines and medical documentation. You will work closely with the health system and develop and maintain relationships with healthcare professionals within the Hospital to identify the appropriate discharge destination.

MAJOR AREAS OF RESPONSIBILITY:

Operations: Confirm complete and timely coordination of health care services and ancillary services according to physician orders. Hold self and team accountable for increased awareness of services offered.

Hiring and Training: Select and onboard staff. Provide oversight for the training and orientation of all PACCs by educating them regarding the Company services i.e.: physician’s offices, long-term care facilities, rehabs, hospitals.

Collaboration: Participate in care conferences and coordination of case management. Ensure all patient, Hospital and staff issues and concerns are fully resolved. Collaborate with PACCs to ensure servicing of account(s) to maintain facility relationships.

Supervision: Directly supervise and manage all department employees and directly manage all employee relations issues.

Discharge Planning: Work with PACC, hospital/facility personnel in the discharge planning process.

Minimize Patient Risk: Promote well-being of patients as a part of the facility team(s).

Policies: Maintain current knowledge of all Medicare/ Insurance / Company and Hospital guidelines, policies and protocol.

Compassionate communicator with a positive attitude

Ability to identify a situation and handle it with the best possible solution with the attitude of doing the right thing for the right reasons

Patience is a virtue when working with patients, families, physicians, and coworkers

Attention to detail is critical, as is being observant and following directions

REQUIREMENTS

Registered Nursing (RN) License in the State of Services

Multi-site Supervisory experience, effective communication skills, a high degree of creativity, and a high degree of decision making

Healthcare experience is desirable

Capable of all physical demands.
