Registered Nurse (RN) – Hospice Inpatient Unit | PRN Nights
Ohio’s Hospice is seeking a compassionate and skilled Registered Nurse (RN) to join our Hospice House/Inpatient Unit team on a PRN basis This overnight position is an excellent opportunity for a nurse who is passionate about providing high-quality, patient-centered end-of-life care in a supportive, collaborative environment.

- Night shift: 6:30 p.m. – 7:00 a.m.
- Every third weekend rotation required

What You’ll Do
As a Hospice RN, you will provide comprehensive nursing care while supporting patients and families through every stage of the hospice journey. Responsibilities include:
- Provide nursing care using the nursing process, including assessment, planning, implementation, and evaluation.
- Assess patients’ physical, emotional, psychosocial, and spiritual needs.
- Develop individualized care plans with patients, families, and the interdisciplinary team.
- Review and update care plans, medications, interventions, and visit frequency as patient needs change.
- Monitor patient conditions and communicate significant changes to physicians and the interdisciplinary team.
- Assist with determining the appropriate level of hospice services based on patient needs.
- Participate in hospice admissions, interdisciplinary team (IDT) meetings, and care conferences.
- Accurately document assessments, physician orders, interventions, and clinical updates in the electronic health record.
- Educate and provide guidance to caregivers, Personal Care Specialists, volunteers, and other members of the care team.
- Complete timely documentation in accordance with Ohio’s Hospice standards.
- Provide effective shift-to-shift communication using SBAR or other approved communication tools.
- Collaborate with other departments and clinical teams to ensure seamless patient care.
- Participate in quality improvement initiatives focused on patient outcomes and organizational performance.
- Assist with onboarding and mentoring new clinical team members as assigned.
Qualifications
- Graduate of an accredited Registered Nursing program; BSN preferred
- Current, unrestricted Registered Nurse (RN) license in the State of Ohio
- Minimum of 2 years of acute care nursing experience preferred
- Hospice and Palliative Nurse Certification (CHPN) preferred Ohio’s Hospice supports eligible employees interested in pursuing certification.
- Current CPR certification.
- Strong computer skills and the ability to accurately document in an electronic medical record.
- Valid driver’s license and safe driving record.
- Current automobile insurance meeting state minimum requirements.
- Ability to safely travel in varying weather and driving conditions when required.
