What You'll Be Doing


  • Provide skilled
    home health nursing visits

    in patients' homes

  • Complete comprehensive assessments and ongoing evaluations

  • Perform
    wound care

    , medication management, injections, and disease monitoring

  • Develop and update individualized care plans

  • Educate patients and caregivers to improve health outcomes

  • Coordinate care with physicians and interdisciplinary team members

  • Document visits accurately in
    Homecare Homebase

  • Identify changes in patient condition and intervene appropriately


Who We're Looking For


  • Active Minnesota Registered Nurse (RN) license

  • Experience in
    home health

    , hospital, rehabilitation, skilled nursing, or acute care

  • Strong assessment, critical thinking, and clinical decision-making skills

  • Compassionate communicator who enjoys educating patients and families

  • Ability to work independently while managing a daily schedule

  • Reliable transportation and willingness to travel throughout the
    St. Paul

    service area
