✨ YOUR ROLE
You are the clinical lead for home-based care.
• Complete in-home patient assessments and ongoing evaluations
• Develop, update, and manage individualized care plans
• Supervise and support CNAs, GNAs, HHAs and Med. Techs.
• Monitor patient condition changes and coordinate follow-up care
• Communicate with families, physicians, and care teams
• Ensure safe, compliant, high-quality care delivery
• Maintain accurate clinical documentation
• Provide education and guidance to caregivers

• Active Maryland RN license (or compact license eligible in MD)
• CPR/First Aid certification
• Reliable transportation for home visits
• Strong clinical assessment and communication skills
• Home care, hospice, or geriatric experience preferred

Compensation: $40.00 - $42.00 per hour
