What You'll Be Doing

• 
  Provide skilled
  home health nursing visits
  throughout multiple service areas
• 
  Perform comprehensive patient assessments
• 
  Deliver
  wound care
  , medication management, injections, and other skilled nursing services
• 
  Educate patients and caregivers on diagnoses and treatment plans
• 
  Collaborate with physicians, therapists, and interdisciplinary care teams
• 
  Float between territories to support census growth, vacations, and staffing needs
• 
  Complete timely documentation using
  Homecare Homebase (HCHB)
• 
  Help ensure continuity of care for patients across the organization

Who We're Looking For

• 
  Active Minnesota Registered Nurse (RN) license
• 
  Experience in
  home health
  , hospital, rehabilitation, skilled nursing, or acute care
• 
  Strong clinical judgment and assessment skills
• 
  Comfortable adapting to different territories and patient populations
• 
  Excellent communication and organizational abilities
• 
  Reliable transportation and willingness to travel throughout the
  Twin Cities Metro
