POSITION PURPOSE
Registered Nurse, Nurse Coordinator for Michigan Heart's Triage department.
This position is a Full-Time, scheduled 40 hours per week on the day shift. (6:30 a.m. to 3 p.m. or 7:30 a.m. to 4 p.m.) No Weekends, Holidays, Nights or On-Call.
Position is full-onsite at Trinity Health Ann Arbor with occasional travel to other SEMI locations.
Leading a team of 20 nurses and medical secretaries the Nurse Coordinator assumes delegated accountability from the Nurse Manager for operations, scheduling and staffing for a specified period. The Nurse Coordinator demonstrates proficiency in working with complex patients according to the unit's scope of practice and serves as a unit resource and leader. He/she demonstrates communication and conflict resolution skills for staff. Role model for Relationship Based Care.

MINIUMUM QUALIFICATIONS
Licensure: Current Michigan RN License
Education: Graduation from accredited nursing program with a Bachelor of Science degree in Nursing
Experience: Three or more years of progressive Clinical Nursing experience in Cardiovascular health.
WHAT YOU WILL DO
Ensures the availability, access and timeliness of Triage services in order to optimize quality of care, coordination, and patient.
Utilizes advanced training and education to provide complex nursing care/specialty care.
Serves as a nurse leader by assisting management team with hiring, motivating, and engaging clinical staff.
Identifies and targets at-risk patients who require extensive care to improve health outcomes.
May conduct visits at all Michigan Heart sites, including Ann Arbor, Livingston, Canton, Chelsea, and Livonia.
Collaborates with office leadership on the development and review of office/program clinical workflows to meet the needs of the patient population.
Supports the development of divisional clinical protocols.
Develops and leads quality initiatives and performance improvement projects specific to the patient population, practice, or division/specialty.
Assists in identifying issues and provides solution suggestions.
Serves as a team leader in the absence of a manager and may support management duties.
Performs/Assists with any projects, pilots, initiatives or duties as assigned and works within scope of RN licensure.
Works with patients, providers and caregivers to identify possible barriers to care and systematically helps to break down barriers (i.e. Financial, transportation, language, etc.).
Facilitates as necessary, timeliness of appointments; works continuously to reduce the intervals between time of abnormal results, diagnosis and treatment.
Provides and/or facilitates clinical and emotional support, education and resources to patients and their families.
Facilitate tests for patients; Prepare patient office records for patient appointments
