$85,000 – $130,000 annually

What You'll Do

• 
  Manage a home health caseload throughout
  Woodbury, Cottage Grove, Hastings, St. Paul Park, and Landfall.
• 
  Complete comprehensive OASIS assessments and individualized care plans.
• 
  Deliver skilled nursing care and patient education.
• 
  Collaborate closely with physicians and interdisciplinary team members.
• 
  Help patients recover, remain independent, and avoid unnecessary hospitalizations.
• 
  Maintain accurate clinical documentation.

What We're Looking For

• 
  Active Minnesota
  Registered Nurse (RN)
  license.
• 
  3+ years of RN experience
  (home health experience preferred).
• 
  Strong assessment and critical thinking skills.
• 
  Excellent communication and organizational abilities.
• 
  Reliable transportation for local travel.
