You'll help patients recover after hospitalization, manage chronic illnesses, prevent complications, and remain safely at home while providing compassionate, individualized nursing care.

What You'll Be Doing

• 
  Provide skilled
  home health nursing visits
  throughout the
  St. Paul
  service area
• 
  Perform comprehensive patient assessments
• 
  Deliver
  wound care
  , medication management, injections, and other skilled nursing services
• 
  Educate patients and caregivers on diagnoses, medications, and treatment plans
• 
  Monitor patient progress and communicate changes with physicians
• 
  Collaborate with therapists, social workers, and interdisciplinary care teams
• 
  Complete timely documentation in
  Homecare Homebase (HCHB)
• 
  Promote positive patient outcomes while helping reduce unnecessary hospitalizations

Who We're Looking For

• 
  Active Minnesota Registered Nurse (RN) license
• 
  Experience in
  home health
  , hospital, rehabilitation, skilled nursing, or other clinical settings
• 
  Strong assessment and critical thinking skills
• 
  Excellent communication and relationship-building abilities
• 
  Ability to work independently while managing daily visits
• 
  Reliable transportation and willingness to travel throughout the
  St. Paul
  territory
