Job Description

FlexStaff is seeking a Registered Nurse with experience in home health agency (CHHA) or long-term homehealth care (LHCSA)to work as a Clinical Care Manager for one of our external clients, a non-profit organization that provides home healthcare services to the elderly, disabled and chronically ill in the New York metropolitan area.

  • Schedule: Monday - Friday 8:30am-5:00pm (1 hour lunch) (FT)Weekly Hours: 37.5

  • Pay rate: $73 per hour

  • Location: Bronx, NY

  • Temporary contract with potential to be hired permanently

In this role you will be coordinating, facilitating, and arranging long-term care services innursing homes, rehab facilities, etc. as needed.

Requirements:

  • Degree from an NLN accredited nursing school (RN, BSN required or atleast five (5+) years’ experience in health care setting).
  • Current active and unrestricted license and registration in New York State as a Registered Nurse required.

  • Minimum of two (2+) years of administrative experience in a managementcapacity in a certified home health agency (CHHA) and/or long-term homehealth care (LHCSA).

Responsibilities include, but not limited to:

  • Oversee development and execution of individualized care plans to improve satisfaction and outcomes.

  • Lead and document interdisciplinary team meetings; complete attestations; support Virtual interdisciplinary team.

  • Serve as manager on call after hours; complete follow-ups, documentation, and initiate grievances promptly.

  • Run Clinical Delivery reports.

  • Allocate staffing and resources to ensure seamless coverage and operations.

  • Coordinate ongoing communication with IDT and stakeholders to align care with participant goals.

  • Review UAS/UAS analyzer with clinicians to ensure assessment accuracy.

  • Investigate incidents and complete Level 2 reviews and documentation per regulations.

  • Ensure complete, accurate participant documentation (enrollment, disenrollment, EMR, clinical notes) and perform chart audits.

  • Coordinate transitions of care and arrange long-term care services as needed.

  • Monitor care management effectiveness/utilization and adjust plans accordingly; ensure compliance with organizational, state, and federal standards.

  • Drive quality improvement; stay current on clinical guidelines and practice standards.

  • Partner with HR on hiring, coaching, onboarding, training, and regulatory compliance; evaluate staff performance and address grievances.

  • Serve as LHCSA Director of Patient Services (DPS) as assigned; practice within scope; meet PACE competencies; active IDT member.

  • Perform other duties as assigned.
