As an RN Case Manager at Canyon Vista Medical Center, you will play a vital role in guiding patients through their healthcare journey while serving as a key connection between patients, families, physicians, and insurance providers. Our case management team works collaboratively across the Medical-Surgical, Telemetry, and Intensive Care units to coordinate care, advocate for patients, and ensure safe, successful transitions from hospitalization to home or the next level of care. Based on Canyon Vista Medical Center's Case Management program, nurse case managers are deeply involved in discharge planning, care coordination, resource management, and daily interdisciplinary collaboration to support quality patient outcome.
The RN Case Manager monitors and manages patient care to promote continuity of care, optimal outcomes, patient satisfaction, cost efficiency, and regulatory compliance. Collaborates with the multidisciplinary team, payors, and community resources; leads discharge planning and utilization review activities. Reports to the Director or Manager of Department.
