Job Title: RN Case Manager | Northwest Metro & St. Cloud Home Health | $6,000 First-Year Bonus Company: Home Health Care, Inc. As an RN Case Manager with Home Health Care, Inc. , you won't simply provide an isolated nursing visit and move on. You'll take ownership of a patient caseload and help guide patients from their initial home health assessment through recovery, stabilization, and eventual discharge. You get to see the progress your nursing helped create. Become the Nurse Who Knows the Whole Story A patient comes home from the hospital with a new diagnosis, medication changes, therapy orders, and a stack of discharge instructions. What happens next? That's where you come in. As their RN Case Manager, you're helping connect all the pieces. You understand why they were hospitalized. You know what medications they're taking. You know what their physician is concerned about. You know what therapy is working toward. You know what the family is struggling with. And because you continue following the patient, you know when something has changed. That's what makes Case Management different. You're not simply completing nursing tasks. You're overseeing the clinical picture. Your Territory: Northwest Metro to St. Cloud We're looking for an RN to help serve patients throughout the northwest Twin Cities and St. Cloud corridor . Our service area includes communities such as: Plymouth, Buffalo, Monticello, St. Cloud, Becker, Big Lake, Clearwater, Maple Lake, Annandale, Rockford, Medina, Corcoran, and surrounding communities. Patient scheduling and territory assignments are designed around geographic needs, with visits generally concentrated within approximately a 35-mile radius of your home whenever possible . What You'll Own As an RN Case Manager, you'll be responsible for coordinating and overseeing an assigned home health caseload. That includes: Managing an assigned home health patient caseload Completing OASIS Start of Care, Recertification, and Discharge assessments Performing comprehensive nursing assessments Developing and updating individualized plans of care Providing skilled nursing visits Monitoring patient progress throughout the episode of care Recognizing changes in condition and intervening appropriately Providing medication management and patient education Performing wound care and other skilled nursing services Communicating with physicians regarding clinical concerns and changes Coordinating with PT, OT, SLP, social work, and other interdisciplinary team members Educating patients and caregivers throughout recovery Helping prevent avoidable emergency department visits and rehospitalizations Completing timely documentation using Homecare Homebase (HCHB) You're the Clinical Quarterback of the Case. Home health Case Management requires a nurse who can see beyond today's visit. What needs to happen next week? Is the patient progressing toward their goals? Are there barriers keeping them from improving? Does the physician need an update? Does therapy need to be involved? Does the family understand the plan? Is the patient actually safe at home? You're constantly connecting the dots. And when everyone involved in the patient's care needs to understand what's happening, you're often the person helping bring that information together. We're looking for: An active Minnesota Registered Nurse (RN) license Three or more years of RN experience Home health experience preferred OASIS experience highly valuable Strong comprehensive assessment skills Excellent clinical judgment and critical thinking Strong organization and time-management abilities Confidence managing multiple patients and priorities Excellent physician, patient, caregiver, and interdisciplinary communication Ability to work independently in a field-based environment Reliable transportation for patient visits throughout your assigned territory Experienced nurses from hospital, acute care, rehabilitation, skilled nursing, and other clinical environments are also encouraged to connect with us.
