The Registered Nurse (RN) Case Manager will utilize a holistic approach for client services. Services include assessment, treatment planning, and implementing care following the bio psychosocial model, in accordance with established policies and practices.

The RN Case Manager will provide addiction medication and team-based care to clients with substance use/co-occurring disorders in a variety of community settings, including at Rapid Access to Addiction Medicine Clinics (RAAM). The RN Case Manager will also provide out-reach for withdrawal management at various locations.

The position involves working collaboratively within a multi-professional team including Nurse Practitioners, addiction / mental health counselors, physicians, psychiatrists, emergency departments, family health teams and other community health service providers. Services may be delivered virtually, telephone & in-person, and outreach in community settings. The position will include Community Withdrawal Management supports for persons with addiction concerns.

WHAT WE'RE OFFERING:

  • What you'll be making - Starting rate for this role is $32.13 per hour (Level 1 – RN/OT),
    • Successful external applicants will start at Level 1, with grid progression based on seniority up to Level 8 ($41.41 per hour); for internal applicants, grid level placement will occur as per the Collective Agreement
  • When you'll work - 70 hours over two-week period, Monday to Friday, 8:30am-4:30pm
  • Your pension plan –Immediately enrolled in the Healthcare of Ontario Pension Plan (HOOPP)
  • What your benefits will be – After a 90-day waiting period, will be enrolled in the extended healthcare benefits plan


HOW YOU WILL BE MAKING A DIFFERENCE:

Direct Service

  • Responsible for screening, assessment and provision of nursing interventions and supports to individuals in the community
  • Collaborate with mental health and addiction service providers to support the mental health, addiction and primary care health needs of clients who may be in crisis or involved with community mental health and addiction agencies
  • Supporting intake services
  • Providing addiction medicine in the community
  • Administer Urine Drug Screens
  • Use virtual technology to connect with individuals who experience barriers to system and treatment access
  • Promote telemedicine to healthcare professionals, clients and the community
  • Assist with program development, implementation and evaluation

Duties specific to RN, Case Manager

  • Provide on-going system navigation and case management support short-term to individuals needing step up care, more intensive supports outside of RAAM and COAP programs

WHAT YOU'LL NEED TO APPLY:

Police Vulnerable Sector Check Requirements

Successful candidates must have a Police Vulnerable Sector Check completed in the last six months. Interested applicants who reside in the City of London and are in need of a Police Vulnerable Sector Check (PVSC) are encouraged to apply for one as soon as possible as PVSCs in the City of London are currently taking approximately 5 weeks to be processed and offers of employment cannot be made without receipt of a PVSC.

We require the following qualifications:

  • College diploma or University degree in Nursing(primary proof of credentials will be required during the hiring process i.e. original degree/diploma or official transcript)
  • Registered Nurse (RN) in good standing with the College of Nurses of Ontario (CNO)
  • Current, satisfactory Police Vulnerable Sector Check (PVSC)
  • Access to a vehicle required for personal transportation
  • Demonstrated ability to attend work regularly

We prefer the following qualifications:

  • Bilingual both official languages, preferred; préférence sera accordée aux candidats qui sont compétents dans les deux langues officielles du Canada
  • Bachelor of Science in Nursing (BScN)
  • Has purchased liability protection under RNAO or independently
  • ASIST training preferred
  • Non-Violent Crisis Intervention training preferred
  • Current First Aid/CPR certification
