At Good Samaritan Hospice, we consider it a sacred honor to walk alongside individuals and families during the end-of-life journey. We are seeking a compassionate, grounded, and skilled Registered Nurse Case Manager to provide expert clinical care, guidance, and comfort during some of life’s most profound moments.

If you are looking for a role where your clinical expertise truly matters, where relationships are valued, and where you can see the impact of your care every day, this may be the perfect fit.

What You Will Do

  • Serve as a key member of the Interdisciplinary Team (IDT), providing holistic, patient- and family-centered hospice care
  • Complete comprehensive nursing assessments, admissions, and ongoing reassessments in accordance with hospice regulations and timelines
  • Develop, implement, and continuously evaluate individualized plans of care in collaboration with the IDT and attending physicians
  • Provide skilled nursing care, including symptom management, pain control, wound care, IV therapy, and physical assessments
  • Educate patients and families on disease progression, medications, pain and symptom management, and routine caregiving tasks
  • Provide emotional support and compassionate guidance to patients and families throughout the hospice journey
  • Fulfill on-call responsibilities as assigned and assist with admissions or coverage during staffing needs

  • Minimum of two (2) years of nursing experience
